EXPLANATION OF BSN & CRUKS

In addition to a valid passport, we also need your Citizen Service Number (BSN). This number is on your health insurance card, which you receive when you are employed in the Netherlands.If you do not have a health insurance card, you must sign our General Terms and Conditions. On this form, you indicate that you do NOT have a BSN number and that you arenot registered in the CRUKS register. If there are any changes in your situation or your details, it is your responsibility to inform us immediately when you visit our casino. 

The employee will then record your full name, date of birth, and(if available)place of birth in our visitor registration system.
